1.Introduction

Welcome to Form Health Club! We’re delighted to have you as part of our community. 

Form Health Club is operated by Form Health Club Pty Ltd under ABN: 83 680 407 498 operating at 587 Pacific Highway, Belmont NSW 2280.

Before you engage in any activities or utilise our facilities, please take a moment to review these Terms and Conditions. They outline the rules and guidelines for your participation and use of our services. By accessing or using Form Health Club’s facilities and services, you agree to comply with and be bound by these Terms and Conditions. If you are under 18, you are considered a minor and must have a parent or guardian over 18 acknowledge these terms and conditions on your behalf. All minors are required to review and sign Form Health Club’s Liability Waiver.

Form Health Club reserves the right to modify our terms and conditions at any time, at its discretion, with reasonable notice. You will be informed of any changes via email, phone call or text message should your contact details be provided.

Form Health Club also reserves the right to deny entry to or remove you from any of our Clubs for any reason.

2. Definitions

"Form Health Club" refers to the fitness and wellness facility operated by Form Health Club, including its physical locations, services, and any associated online platforms.

"Member" refers to any individual who has completed the registration process and has been accepted as a member of Form Health Club, including any authorised users under their membership.

"Minor" refers to any individual under the age of 18. Minors must have a parent or guardian sign on their behalf and provide necessary contact details.

"Services" includes all fitness, wellness, and related activities provided by Form Health Club, including but not limited to classes, personal training sessions, and use of facilities.

"Facilities" refers to the physical locations and equipment provided by Form Health Club for the purpose of fitness and wellness activities.

"Terms and Conditions" refers to this document and any additional terms, conditions, or policies published by Form Health Club that govern the use of its services and facilities.

"Fees" encompasses all charges associated with membership, services, and any other related costs as determined by Form Health Club.

"We," "Us," or "Our" refers to Form Health Club and its employees, representatives, and agents.

"You" or "Your" refers to the Member or any individual accessing or using Form Health Club’s services and facilities.

3. Booking & cancellation Policy 

We strongly recommend you pre-book your classes online either via the Form Health Club app or website to reserve your place. Bookings open 14 days in advance. 

We have a 4 hour cancellation policy for all PM classes and a 12 hour cancellation policy for all AM classes. We have a 12 hour cancellation policy for private sessions.

Thank you for respecting our waitlisted clients and honouring these cancellation windows. In order to avoid forfeiting your class or getting a late cancellation charge, please be sure to cancel on the app prior to the late cancellation window times, specified above.

Late Cancellation fees and no show fees are $10 per class for membership holders and $30 per private. Casual passes or Class Packs will be forfeited for the applicable class if you cancel late or no show.

4. Waitlist Terms 

If a class is full, you can place yourself on the waitlist if there is a spot. The waitlist will move down in order of client bookings. If you are the next to be placed into the class, you will be automatically added and will receive a text message to confirm this. Please ensure you remove yourself off any waitlist if you can no longer attend that class. For classes starting within 60 minutes you’ll be asked and will need to confirm if you would like to join should you still be on the waitlist and a spot in the class arises.

5. Class Etiquette 

  • Please arrive 10 minutes early to your class out of respect for the instructor and other clients 

  • Leave all larger belongings in our storage area - we want a safe and clutter free zone - also a great time to unplug from technology!

  • Please place mobile phones on silent if you choose to bring it into class with you (however we recommend leaving them in our storage area)

  • For safety reasons, Grip socks are required for all reformer classes

  • No food is permitted in the studio rooms - water is recommended

  • You may decide to bring a sweat towel with you

  • Please ensure you are checked in by the instructor at every class, if you aren’t checked in this may result in a no show fee being charged

  • Please stay for the entire duration of class. Any extenuating circumstances needs to be communicated with your instructor prior to class

  • If you arrive more than 5 mins late, you may be turned away from participating in the class for the safety and enjoyment of all other clients. Please let us know via email, text or in-app messaging if you are running late.

6. Recovery Session Etiquette

  • Please arrive 5 minutes before your session time to ensure a seamless experience and to get the most out of your 45 minute session. 

  • Please bring with you a towel to wipe down after, and we will supply a towel to sit on during your session. If you forget your towel you can hire them for $5 each

  • Clean the recovery room with equipment provided following the guidelines after your session

  • Please respect other clients by ensuring you stick to your session times and depart the recovery room on time

  • Please ensure the recovery room is left in a clean and tidy state ready for the next booking

  • Please do not use the recovery room if you are unwell, injured or have any health conditions that could impact your ability to be safe and/or further exacerbate your injury or illness 

7. Member Payment Terms

Payments are processed weekly, beginning the day your membership is created. Your payment covers classes from that day of the week, for 7 days. All membership policies below apply.

8. Member Cancellation Terms

All memberships have a minimum 13 weeks term (unless purchased with special conditions as per below). After the initial minimum 13 week term, you can request to cancel your membership by emailing us at hello@formhealthclub.com.au with at least 4 weeks notice. You will receive an email reply confirming your cancellation. 

For our 6 month discounted memberships, the full term must be completed prior to canceling. If you are a 6 month member and wish to cancel before the end of your term, you must pay 50% of the remaining dollar value of your membership. A 4 week notice period for cancellations still applies.

We do understand that life can throw you a curveball, so please discuss your situation with us directly if need be, in writing to hello@formhealthclub.com.au

9. Membership Changes

You are welcome to upgrade your membership at any time during your membership term.

In the event you wish to reduce your membership, you must have fulfilled your 13 week commitment to change your membership.

Any change to your membership does enact a new minimum 13 week membership term. Simply email hello@formhealthclub.com.au to request an upgrade or downgrade to your membership.

10. Membership Freezes

You are welcome to pause your membership at any time, for a minimum of 7 days. You may do this once every 3 months with no fees. Additional suspensions will be charged at the rate of $10 per week. If you would like to suspend your membership simply email us at hello@formhealthclub.com.au with at least 7 days notice from your next scheduled direct debit payment.

11. Two Week Intro Pass

The introductory offer is only valid for new clients who have not attended Form Health Club prior. The offer can be used once only per client and is valid for 14 days from the date of your first class booking. We are unable to pause, refund or extend intro passes for any reason.

12. Retail Returns Policy

Retail items may be exchanged if returned in their original, undamaged condition. We cannot refund any retail items due to a change of mind.

13. Belongings & Valuables

Form Health Club will not accept responsibility for any lost or damaged valuables brought to the Club. We will always try our best to locate any misplaced items for you, so please reach out hello@formhealth.com.au

14. Notification of Changes

You must inform Form Health Club of any changes to my contact information, bank accounts, or credit card details. If payments or fees are overdue, Form Health Club may suspend my membership until all debts are settled. Continued non-payment may result in membership cancellation.

15. General

Form Health Club reserves the right to cancel and change instructors, events and classes without prior notice. If a client is impacted due to a class change they will be contacted in writing.
